Physicals will take place Monday, April 22, 2024 in the CHS Cafeteria for all incoming 7-12 grade students. They will start at 3:00 PM and last until the last person is in line. The cost is $15 cash ONLY. All participants must wear shorts.
Attached is the pre-participation physical form. A physical must be completed with a medical doctor prior to participation in athletics.
